MyChart proxy access allows you to view and manage the health information of a loved one—such as your child, aging parent, or someone you care for—securely and conveniently.

FAQs
Patients age 13 and older can have their own MyChart account.
Ages 13–17 (Minors): Teen patients can access their own MyChart account but cannot invite others to view it. To grant a parent or legal guardian access, both the minor and the guardian must complete and sign the MyChart Proxy Access Form for Minors (see links below).
Ages 18 and older (Adults): Adult patients can manage access to their MyChart account. They can send proxy invitations directly through MyChart or complete a MyChart Proxy Access Form for Adults (see links below) to authorize someone to help manage their care. Proxy access can be removed by the patient at any time.
Patients 12 years old and under: Through your MyChart portal, access Questionnaires from the Menu Section and complete the MyChart Proxy Patient Information Questionnaire.
Adult patients and minor patients aged 13-17: Download and complete the appropriate form below. Through your MyChart portal, access Questionnaires from the Menu Section, complete the MyChart Proxy Patient Information Questionnaire and upload the completed MyChart Proxy Access Form.

Log in to your MyChart account using your MyChart username and password.
From your dashboard, click on the name of the person whose account you have proxy access to. This will switch you into their MyChart view, where you can manage their health information.
