At Health First, we’re guided by one mission: to improve the health and wellness of the communities we serve.
Through our Wellness Grants program, we partner with local nonprofit organizations across Brevard County to expand access to care, strengthen community resources, and support those who need it most. These grants reflect our priorities—improving access to healthcare, expanding opportunities for physical activity and nutrition, and supporting our neighbors in times of need.

What are Wellness Grants?
Wellness Grants are designed to support nonprofit organizations addressing priority health needs identified through our Community Health Needs Assessment (CHNA).

How to apply for a Wellness Grant.
Wellness Grants support programs that improve health outcomes for Brevard County residents, guided by Health First’s Community Health Needs Assessment (CHNA).
- Applications must be submitted within the designated application window. Late, incomplete, or ineligible applications will not be considered.
- All proposals are reviewed through a standardized evaluation process to ensure consistency and fairness. The highest-scoring applications will be invited to present to the Community Health Improvement Committee (CHIC), which makes final funding decisions.
- Organizations selected for funding are required to submit quarterly reports demonstrating progress toward program goals and use of funds.

- Be a 501(c)(3) non-profit organization (or equivalent).
- Serve residents of Brevard County.
- Propose a program aligned with at least one priority or area of interest:
- Nutrition, physical activity, and weight management.
- Access to healthcare services.
- Food insecurity.
- Housing instability.
Submit a complete application by the deadline.
Programs focused on student populations are supported through Health First’s partnership with Brevard Public Schools. Private schools and private educational institutions are not eligible.
